What is double chin liposuction?

The short answer

Double-chin liposuction removes the fat under the chin and along the jaw line through a small incision. At Rewind it is almost never the whole operation: the clinic treats the blurred border between jaw and neck as a problem of three things at once — the fat, the platysma muscle that has slackened with age, and, in some patients, an enlarged salivary gland beneath the jaw.

Which of those three is driving your profile is a question of examination, not of preference. The surgeon decides it before anything is booked, and the answer changes both the operation and the price.

Which layers does the operation actually reach?

A double chin is not one deposit of fat sitting in one place. The Rewind operation is described layer by layer, and each layer is addressed by a different manoeuvre — which is why the plan cannot be settled from a photograph.

Subcutaneous fat
The fat between skin and muscle, under the chin and along the jaw line. This is the layer that liposuction reaches through the incision.

Grape-cluster fat
Rewind's own term for the fat that clings to the neck muscle in beads. It cannot be suctioned; it is extracted piece by piece, along with the deep fat lying inside the muscle.

The platysma
The broad sheet of neck muscle. When it slackens with age the neck creases and the jaw-to-neck border softens, and tightening it is a separate step from removing fat.

Submandibular gland
A walnut-sized saliva-producing organ sitting under the jawbone. Where it is enlarged and pushes below the jaw line, part of it may be reduced.

How is double chin liposuction performed?

Rewind publishes the operation as six steps. They run from the shallowest layer to the deepest and back to the surface, and not every patient needs all six — the examination decides which steps belong in your plan.

01Access

A small incision, then the fat layer

The operation begins with a minimal incision placed to keep scarring small. Through it the subcutaneous fat layer is suctioned — the part of the procedure most people picture when they say liposuction.

02Extract

The beads on the muscle come out by hand

The grape-cluster fat stuck to the neck muscle will not travel through a cannula. It is taken out one piece at a time, which is slower work and the reason a suction-only result can look unfinished.

03Deeper

Then the fat inside the muscle

Beneath and within the platysma sits a further deposit. Removing it is what changes the depth of the angle between chin and neck beyond the surface fullness alone.

04Tighten

The slackened platysma is tied back

The muscle that has stretched is drawn up and secured under tension. This is the step Rewind names in Korean as the double-chin muscle tie, and it is the part of the operation that addresses neck creasing more than volume.

05Reduce

Where the gland protrudes, part of it is reduced

If the submandibular gland bulges below the jaw line, a portion is removed. Because this step touches a working organ, not a fat layer, it is only ever taken after the surgeon has examined the area and explained what it involves.

06Close

The incision is sutured

The access point is closed and the compression dressing goes on. Rewind records a scar of roughly 2 cm under the chin and about 0.5 cm behind each ear, which it describes as barely visible once healed.

Why here

Why have double chin surgery at a single-surgeon clinic?

The reason a double chin comes back is rarely that too little fat was taken. It is that the wrong thing was treated. Rewind is built around one surgeon who examines, operates and follows up, so the diagnosis and the operation are never made by different people.

Dr. Choi examines the submental area himself and tells you which of the three is producing your profile. Where suction alone would leave the line unchanged, he says so before you book, not afterwards.

Rewind is a single-surgeon practice on the fourth and fifth floors of the Richro Building in Apgujeong. The operation is planned at consultation by the medical director and carried out by him — there is no second operator to whom a case can be handed over.

Who is double chin liposuction for?

Often a good fit

  • Skin laxity as the dominant finding — a lift, not suction, may be the plain answer
  • Any plan involving the salivary gland, which is a working organ and not simply tissue to remove
  • A short or receding chin structure, where the profile is partly skeletal

Recovery

What is recovery after double chin surgery like?

This is surgery, and the aftercare is where a good result is either kept or lost. Rewind's own post-operative instructions are short and specific, and two separate sheets apply here — one for facial liposuction, one for the platysma tightening. Where they differ, follow the longer of the two. The clinic records bruising and swelling as expected, with an early return to ordinary daily life, and describes the durability of the result as more than five years, varying between individuals.

Week one
The dressing stays on
A compression dressing is kept in place for one week. Keep the wound clean — Rewind asks patients to come in at once for severe pain or a fever.
First weeks
Bruising and swelling
Both are expected and settle. Compression and cold compresses help; do not massage the operated area or press on the muscle while it is healing.
2–4 weeks
No alcohol, smoking or sauna
The platysma instructions ask for at least two weeks; the facial liposuction sheet extends alcohol, sauna and strenuous exercise to one month. Take the longer figure.
One month
Careful with the neck
For a month, avoid dropping the chin sharply or turning the head hard to either side — the movements that pull on exactly what was tightened.

Any surgical procedure carries risk, including bleeding, infection, swelling, altered sensation and scarring. Rewind records a scar of roughly 2 cm under the chin and about 0.5 cm behind each ear. Tell the surgeon about every previous procedure in this area, and about any medication you take, before the operation is scheduled.

What is double chin liposuction?
It is the removal of fat under the chin and along the jaw line through a small incision. At Rewind it is usually combined with tightening of the platysma, the neck muscle whose slackening blurs the border between jaw and neck.
Why is liposuction alone often not enough?
Because a double chin is made of more than one layer. Rewind's own position is that fat removal has to be considered together with the platysma, the fat clinging to it and the salivary gland under the jaw before a smooth jaw line can result.
Where is the incision, and will the scar show?
The incision is a minimal one under the chin. Rewind records a scar of about 2 cm under the chin and about 0.5 cm behind each ear, and describes them as barely visible once healed.
What is grape-cluster fat?
It is Rewind's term for the fat that clings in beads to the neck muscle. It cannot be drawn out through a suction cannula, so it is extracted piece by piece during the operation.
Why would the salivary gland be part of a chin operation?
The submandibular gland is a walnut-sized organ beneath the jawbone. When it is enlarged it pushes below the jaw line and keeps the area full even after fat has been removed, so part of it may be reduced — a step taken only after examination and explanation.
How long does the result last?
Rewind records a durability of more than five years, with variation between individuals. Weight change and ordinary ageing continue afterwards, as they do after any contouring operation.
What anaesthesia is used and how long does the operation take?
Rewind does not publish a fixed anaesthesia protocol or operating time for this procedure, so we do not state one here. Both are confirmed by the surgeon at consultation once your plan is set.
How long do I wear the compression dressing?
One week. Rewind asks patients to keep the compression dressing on for seven days after surgery, and compression together with cold compresses is what the clinic recommends for settling the swelling.
When can I go back to normal life?
Bruising and swelling are expected, and Rewind records an early return to ordinary daily activity. Strenuous exercise, sauna, alcohol and smoking are a separate question and are restricted for longer.
What should I avoid after the operation?
Sauna, strong massage, alcohol and smoking — at least two weeks for the platysma work, and up to one month for facial liposuction, where strenuous exercise is included. For one month, also avoid dropping the chin sharply or turning the head forcefully.
Is this the same as a neck lift?
No. This operation removes fat and tightens the platysma through a small submental incision. A neck lift addresses loose skin and is a different procedure with its own listing at Rewind. Which one applies to you depends on whether the finding is volume or laxity.
Who will operate, and can I be seen in English?
Rewind is a single-surgeon practice. The medical director, Dr. Jonghwan Choi, sees every patient at consultation and performs the operation himself. Interpreter-assisted consultations are available for international patients, and Rewind is a KHIDI-registered institution for foreign patients.
