What does platysma tightening add to double chin liposuction?

The short answer

Liposuction takes out fat. Platysma tightening deals with the thin sheet of muscle that runs from the jaw down across the front of the neck. When that muscle has slackened with age, the neck creases and the border between jaw and neck blurs, and fat removal cannot draw it back. In the combined operation at Rewind, the fat is suctioned first through a short incision under the chin, then the slackened platysma is drawn together and tied so the neck carries tension again. The clinic's Korean name for this step translates as the double chin muscle tie.

Put simply, suction changes volume and tightening changes support. A neck that is full but firm needs the first. A neck that is full and also loose at the muscle needs both, because taking fat out of a slack neck can leave the profile softer than expected once the swelling has gone down.

The decision is physical. It is made with the neck tensed in front of the surgeon, and it is one of the few points in contour surgery where you can feel the finding with your own fingers.

Where the suction stops and the platysma tightening begins

The combined operation works on four structures under the chin. Two are fat, one is muscle, and one is a gland that is touched only when it is enlarged.

Suctioned through the short incision under the chin. This is the same layer that liposuction performed on its own would remove.

The platysma
The muscle sheet across the front of the neck. When it slackens, the neck creases and loses the angle beneath the jaw. Tying it back restores tension across that front.

Fat on and within the muscle
The beaded deposit Rewind calls grape-cluster fat, and the deeper fat inside the muscle layer. Where present, it is taken out by hand once the muscle is in view.

The submandibular gland
Partly reduced only where it pushes below the jaw line. It is never a routine part of the muscle work.

How double chin liposuction with platysma tightening is carried out

The six published steps run from the surface down and back up again. Step four is the one that separates this operation from suction alone.

01Access

A short line under the chin

A minimal incision is made beneath the chin, placed to keep the scar small. Rewind records the resulting scar at about 2 cm.

02Suction

The fat layer above the muscle

The subcutaneous fat under the chin and along the jaw is suctioned first. Up to this point the operation is identical to liposuction on its own.

03Deep fat

Fat clinging to the muscle, then fat inside it

Where the examination found it, the beaded fat attached to the platysma is lifted out piece by piece, followed by the fat lying within the muscle layer.

04Tighten

The platysma is drawn together and tied

The slackened muscle is pulled back into tension and stitched so that it supports the tissue under the chin again. This step addresses the neck creases and the soft border between jaw and neck.

05Gland

Gland reduction, only where indicated

If the submandibular gland bulges below the jaw line, part of it is removed. That is a separate decision with its own explanation beforehand.

06Close

Closure and a week of compression

The incision is sutured and a compression dressing is applied, to be worn for one full week.

01Test

The tensed-neck test happens at the chair

Dr. Choi asks you to clench the neck and watches the angle below the jaw. Bands that stand out, or a front of neck that drops forward when relaxed, are what bring the muscle into the plan.

02Restraint

Muscle work is left out when the muscle is sound

Tightening a platysma that has not slackened adds healing time without improving the line. In that situation the plan stays at liposuction, and the reason is explained to you.

03Continuity

The same surgeon checks the tightened muscle afterwards

Follow-up visits are with the person who tied the muscle, so any unevenness or firmness in the healing neck is judged by the surgeon who knows exactly what was done.

Liposuction on its own and the combined operation are separate listings. When the muscle is included, the combined figure already covers the suction, so the two are never added together.

The gap between ₩1,760,000 for suction alone and ₩5,390,000 for the combined operation reflects the added surgical step and the deeper work it involves. Where an earlier chin operation is being corrected, a separate revision listing is used.

Who needs the muscle work

Who needs platysma tightening, and who is fine with liposuction alone

Platysma tightening is usually proposed when

  • Vertical bands appear down the front of the neck when you clench
  • The angle between chin and neck has softened even though the fat layer is modest
  • Neck creases have deepened along with the fullness
  • An earlier liposuction reduced volume but left the profile heavy

Liposuction alone, another operation or no surgery when

  • The fullness is soft fat in front of a firm muscle, so suction alone is the plan
  • Loose, excess skin is the main finding, which belongs to a neck lift
  • The chin bone is set back and is shaping the profile
  • Bleeding or clotting problems, blood-thinning medication, pregnancy or an infection in the area
  • The hope is for a sharply cut jaw line that the underlying structure cannot support

Recovery

Does adding platysma tightening lengthen recovery?

It adds a second set of instructions. Rewind issues separate aftercare sheets for facial liposuction and for the double chin muscle tie, and a combined operation follows both, taking the longer restriction wherever they differ. Bruising, swelling and some tightness are expected, and the clinic records an early return to ordinary daily life.

What changes most is how carefully the neck is moved. The tied muscle heals under tension, so the first month is about avoiding the movements that load it.

Dressing on
The compression dressing stays on for the full week and is not removed early. Keep the incision clean.
Two weeks
The muscle sheet
For at least two weeks: no alcohol, smoking, sauna or steam room, no massage over the neck, and plenty of rest so the muscle is not overworked.
Up to week four
The liposuction sheet
For a month: no strenuous exercise, sauna or alcohol. Compression and cold packs help the swelling. Come in at once for severe pain or fever.
Through week four
Head and chin
For a month, do not drop the chin sharply toward the chest or twist the head hard to either side, since both strain the tied muscle.

Risks of the combined operation include bleeding, haematoma, infection, asymmetry, contour irregularity, temporary numbness, nerve injury and visible scarring, and gland reduction carries considerations of its own that are explained beforehand. Durability is recorded as more than five years, with individual variation.

Platysma tightening with chin liposuction: common questions

What does platysma tightening add to chin liposuction?
Support. Liposuction reduces the fat layer, while tightening draws the stretched neck muscle back into tension so the tissue under the chin is held up again. Rewind combines the two when the examination finds a slackened platysma, which fat removal cannot correct by itself.
Who needs muscle tightening and not just liposuction?
People whose neck shows vertical bands when clenched, whose chin-to-neck angle has softened despite a modest fat layer, or whose earlier liposuction left the profile heavy. A full neck in front of a firm muscle usually needs suction only.
Does adding platysma tightening lengthen recovery?
The restrictions last longer. The muscle aftercare asks for at least two weeks without alcohol, smoking or sauna, the liposuction sheet extends limits on exercise, sauna and alcohol to one month, and neck movement is guarded for a month. The dressing week is the same.
How can I tell whether my platysma has slackened?
Clench your jaw and tense the front of your neck in a mirror. Vertical bands below the chin, or a neck that falls forward when you relax, suggest the muscle is involved. Only an examination confirms it, because fat and skin can produce a similar outline.
Is the same incision used for both parts?
Yes. The suction and the muscle tightening are both carried out through the short incision under the chin, whose scar is recorded at about 2 cm. Rewind also records a small scar of about 0.5 cm behind each ear for this operation, and describes both as barely visible once healed.
Can the muscle be tightened later if I only have liposuction now?
It can be considered, but the neck would then be operated on a second time, and a revision listing applies to an area already treated. Where the muscle is clearly slack at the first examination, doing both together keeps it to one operation.
Does the tightening also help neck creases?
Rewind describes the combined operation as improving neck lines as well as the double chin, since the creases come partly from the slackened muscle. Lines etched deeply into the skin itself may soften without disappearing, and results vary between individuals.
Is the salivary gland part of every combined operation?
No. Gland reduction is a separate step, used only when the submandibular gland pushes below the jaw line. It is explained and agreed on its own before surgery, with its specific risks set out.

Research on managing the platysma in submental surgery

These papers concern submental and neck surgery in general. Kamer and colleagues described the platysmal banding that can show when fat is removed from a neck with a lax muscle; Bravo reports a reduction neck lift working in two planes; Bank examined how skin retracts after liposuction in patients over 40; Aldihani and colleagues report a large series of submental liposuction. None of them studies Rewind patients or the technique used here.
