Is HA filler or micro-fat grafting better for under-eye hollows?

The short answer

Neither is better for everyone. Hyaluronic acid filler is the simpler choice for a clearly defined groove: one appointment, no incision, a change seen the same day, a published duration of six to twelve months at Rewind, and the option to dissolve it with an enzyme. Micro-fat grafting moves your own fat, taken from elsewhere on the body, into the hollow during an operation. It suits wider volume loss around the eye and cheek, part of the graft is reabsorbed, the part that takes stays as living tissue, and it cannot be dissolved. Reversibility is usually the deciding point.

The two options answer the same complaint, a shadowed hollow beneath the eye, with very different materials and commitments. Gel is a manufactured product that sits where it is placed until the body absorbs it. Fat is your own tissue: it has to be harvested, prepared and placed in small amounts so that the surrounding tissue can supply it with blood, and only part of it survives the move.

Neither treats the causes of dark circles that are not about volume, such as brown pigment, veins showing through thin skin or a fat bag that bulges above the groove. The comparison below assumes the examination has already found a hollow; if it finds something else, the conversation changes.

Which hollow you have

Four under-eye hollow patterns and where each points

The shape and extent of the hollow matter more than its depth alone. These four patterns cover most of the faces seen for this question, and each tilts the choice one way or the other.

A narrow, defined groove
A crisp line where the lid meets the cheek, with the rest of the face full. A small amount of gel usually addresses it, and filler is often the first option discussed.

Hollowing around the eye and cheek
Volume loss that runs from the lower lid into the upper cheek. A larger region is involved, and grafting your own fat becomes part of the conversation.

A groove below a fat bag
The hollow sits beneath a bulge. Adding volume from either source can make the bulge look larger, so the bag itself is assessed as a separate eyelid problem.

Very thin or crepey lid skin
Irregularities show through easily. Small, smooth volumes matter with either option, and skin quality may need attention on its own.

01Material

Manufactured gel against your own fat

Filler is a hyaluronic acid gel from a syringe; Rewind stocks one domestic and two imported products. A graft uses fat drawn from a donor site on your body. Rewind does not publish which donor site it uses; that is set at the examination.

02Procedure

An injection visit against an operation

Under-eye filler is given in the treatment room, often on the day of consultation. Fat grafting is surgery: harvest, preparation and placement happen in one session in theatre, on a date set for it. The clinic does not publish anaesthesia or theatre time for either.

03Duration

Gradual absorption against partial take

Gel is absorbed over the published six to twelve months, and published work describes how the body breaks crosslinked hyaluronic acid down. With a graft, some fat is reabsorbed and the rest survives; the proportion varies and no figure specific to Rewind has been published.

04Reversal

An enzyme for gel, no enzyme for fat

Hyaluronidase dissolves hyaluronic acid, and Rewind lists it per area. Once fat has taken, it cannot be dissolved in the same way, so an overfilled or uneven graft is corrected more slowly.

Why weigh under-eye filler against fat grafting with one surgeon?

The under-eye is examined upright and under different light, together with the upper cheek, to see how far the hollow extends. Earlier filler, fat bags and skin quality are noted. If filler is chosen, it can usually be given at the same visit; if a graft is the better route, you leave with a surgical plan, a quote by region and a date.

Appointments run Monday, Tuesday, Wednesday and Friday from 10:00 to 19:00 and Saturday from 10:00 to 16:00; the clinic is closed Thursday and Sunday. For a graft, leave room in your stay for the month of restrictions that follows.

How do cost and longevity compare over three years?

The two are billed on different units, so a single sticker price says little on its own. Under-eye filler is charged per 1cc syringe, and the under-eye takes a small volume; with a published duration of six to twelve months, keeping the result over three years means several treatments, each sized at review. Fat grafting is charged per region as one operation. Rewind's schedule lists a single area, which excludes the forehead and temple, and a separate plan that includes a second session at a price below two separate operations; the portion of the graft that takes is not absorbed the way gel is, though the face keeps ageing around it.

List prices exclude VAT, and where an anaesthetic fee applies it is already inside the figure. Whether an under-eye graft counts as one area or more depends on how far the hollow extends, and that is a finding at the examination.

Who leans toward under-eye filler, and who toward fat grafting?

Under-eye filler usually fits

  • A narrow, defined groove with the upper cheek still full
  • A first treatment, where being able to dissolve the result matters
  • A short stay in Seoul with no room for surgical restrictions
  • No history of puffiness or a bluish tint from earlier gel

Fat grafting usually fits

  • Hollowing that extends across the lid and upper cheek
  • Repeated gel treatments that have left puffiness, or a wish to stop maintaining gel
  • Enough donor fat, confirmed at the examination, and time for a month of restrictions
  • Acceptance that the amount of fat that takes varies, and a second session may be needed

Neither is offered with an active infection near the eye, during pregnancy or breastfeeding, or where a prominent fat bag is the main finding. A history of poor healing, a clotting disorder or blood-thinning medication is discussed before any graft date is set.

After either treatment, report increasing pain, pale or mottled skin, or any change in vision at once. Lumps, asymmetry and prolonged swelling are the more usual problems, and they are reviewed early by the same doctor.

Under-eye filler or fat grafting: eight questions to settle first

Is HA filler or micro-fat grafting better for under-eye hollows?
Neither wins for everyone. Hyaluronic acid suits a defined groove in someone who wants a same-day change that can be dissolved. Micro-fat grafting suits wider volume loss around the eye and cheek in someone ready for an operation and an uncertain amount of take. The examination decides which describes you.
Which option can be reversed if the result is not right?
Hyaluronic acid filler. An enzyme, hyaluronidase, breaks it down, and Rewind lists dissolving per area. Grafted fat is living tissue once it has taken, and no enzyme dissolves it, so correcting an overfilled or lumpy graft is a slower, more involved process.
How do cost and longevity compare over three years?
Filler is paid per syringe and Rewind gives six to twelve months per treatment, so three years means several rounds, each sized at review. Does fat grafted under the eye last longer than filler?
The portion of fat that survives becomes part of the tissue and is not absorbed the way gel is. How much survives varies from person to person, and Rewind does not publish a retention figure, so a second session is sometimes planned. The face also keeps ageing around the graft.
What does recovery look like for each?
After under-eye filler you return to your day at once, with bruising and swelling improving over one to two weeks. After fat grafting, the clinic's sheet also gives one to two weeks for bruising and swelling, plus at least a month without strenuous exercise, alcohol, sauna or steam rooms.
What problems can each option cause under the eye?
Filler can leave bruising, a bluish tint, lumps, puffiness and, rarely, a vascular complication. Fat grafting can cause asymmetry, over- or under-correction, visible lumps, oil cysts, contour irregularity, donor-site changes and rare vascular events. Thin lower lid skin shows irregularities from either.
I already have under-eye filler. Can I switch to fat grafting?
Often, yes. Hyaluronic acid can be dissolved first with hyaluronidase so the surgeon sees the hollow as it really is, and the graft is planned once the area has settled. Tell the clinic the product and date of your earlier filler; gel that is not hyaluronic acid changes the plan.
