Do I need a neck lift or just double chin liposuction?

The short answer

It depends on whether your neck is full or loose. Double chin liposuction removes fat from the layer between skin and the platysma through a short line under the chin, and it relies on your skin shrinking back onto the new outline by itself. A neck lift treats skin that can no longer do that, together with a slackened platysma, by redraping and tightening them. If the skin still retracts, suction is usually the smaller and sufficient operation. If the skin is loose, removing the fat beneath it can leave it hanging, and a lift is discussed.

The two operations are often mentioned in the same breath because they treat the same region, but they work on opposite problems. One takes something away from underneath. The other tightens what lies on top. Choosing between them is mainly a judgement about skin: how thick it is, how it behaves when lifted and released, and how far it has already stretched.

Between the two sits a middle option that Rewind lists separately: chin and jaw line liposuction with the platysma tightened, still through short lines and still without removing skin. Many necks that seem to need a lift turn out to need that instead, and some that seem to need only suction need the muscle work as well.

When double chin liposuction is enough, and when a neck lift is the better fit

Liposuction on its own usually suits

  • Fat you can pinch under the chin, with skin that springs back when lifted
  • A younger neck, or an older one whose skin has kept its thickness
  • A jaw line that reappears when the head is tilted back
  • A stable weight, with no plan to lose a large amount soon

A neck lift, or a pause, is discussed when

  • Skin folds or hangs below the jaw even though there is little fat
  • An earlier liposuction left the neck loose or uneven
  • Weight has dropped a great deal and the skin has not followed
  • Smoking, a clotting disorder, blood thinners that cannot be paused, pregnancy, breastfeeding or an infection make surgery unwise at present

Results vary by individual, and an outcome cannot be promised.

Neck lift or chin liposuction: questions patients ask before choosing

What is the difference between a neck lift and double chin liposuction?
Chin liposuction removes fat from the layer above the neck muscle and relies on skin shrinking back by itself. A neck lift redrapes loose skin and tightens a slack platysma. One treats fullness, the other laxity, and many necks show some of both.
When does liposuction alone leave loose skin?
When the skin has lost the elasticity to contract onto a smaller outline, usually through age, sun damage, thin skin or large weight loss. Removing the fat that was filling it out can then make the looseness more obvious. Lifting and releasing the skin at the examination shows which way it is likely to go.
Am I too old for chin liposuction alone?
Age alone does not decide it. A 1999 paper looked specifically at skin retraction after liposuction in patients over forty, because skin quality varies widely at any age. The surgeon judges your skin by hand and tells you whether suction by itself is likely to be enough.
Can liposuction and a neck lift be done together?
