Does exosome therapy help acne scars, and what does the actual reshaping?

The short answer

Exosome therapy on its own is not a scar treatment. An acne scar is a change in the structure of the skin, and changing structure takes a device that works below the surface. At Rewind that device is Sylfirm X, a radiofrequency microneedling platform, and the exosome vial is combined with it: the pass goes in first and the preparation is spread over the treated skin afterwards. Whether the vial adds anything for your scars is decided at the examination, and it can be left out.

Source: Rewind's price sheet, which lists the exosome vial only in combination with Sylfirm X, and the clinic's equipment record for that device. Neither gives a session count for scarring.

The evidence splits along the same line as the treatment. Fractional radiofrequency microneedling has been studied for atrophic acne scars specifically, including a 2013 study in Asian patients, a 2022 meta-analysis on acne and acne scars, and a 2023 split-face trial that set it against fractional CO2 laser. For exosome products, the papers cited below are broad reviews of skin rejuvenation and of complications. None of their titles describes a scar trial, and that absence is worth knowing before you pay for the vial.

None of this makes the combination pointless. It means the conversation starts with the scar type, the depth and the device, and the vial is weighed at the end.

What you are treating

Acne scar patterns, and where exosome fits in each

Most people arrive with a mixture, so the plan is written zone by zone. The four patterns below cover what is usually seen on the cheeks and temples after acne, and each one points to a different first step.

Soft undulations that catch side light. The pattern that suits a Sylfirm X pass most readily, with a vial considered on top.

Deeper, sharp-edged pits
Scars with steep walls and a firm base. The hardest to change with any needling device, and the plan says so from the start.

Red or brown marks
Flat discolouration left after a breakout has healed. Colour is treated as a separate complaint, often with a laser.

01Below

The problem sits under the surface

An atrophic acne scar forms where inflammation destroyed tissue and the repair left a dip. Nothing spread on top of the skin reaches the layer that holds that dip in place, however it is packaged or priced.

02Depth

Sylfirm X is set to the depth of the scar

The needles on this platform adjust from 0.3 mm to 4.0 mm, according to Rewind's equipment record, and the heat is released at the tip. For scarring the working depth is set against the floor of the scars in each zone. Two areas of the same face can therefore be treated at different depths.

03Repair

Remodelling is gradual and uneven

Controlled heat at depth sets off a repair response that continues after the visible redness has gone. Scars rarely change evenly, and some patterns barely move. This is the part of the process that the published microneedle radiofrequency studies measure.

04Vial

What the exosome vial is asked to do

Applied over the freshly treated field, the preparation is added with the recovering skin in mind. The reviews below describe exosome products for skin rejuvenation in general. Claims that it fills scars go beyond anything in those titles, and you should hear that plainly at consultation.

How Rewind plans acne scar treatment around exosome

Scarring is a long project, and a good plan is modest about what each step can change. Three principles shape how the vial is used for scars at this clinic.

01Order

Active acne is calmed first, scars second

Needling through inflamed spots risks spreading infection and making marks worse. When breakouts are still active, the first plan addresses them, and scar work is booked for later. Rewind's sheet lists a separate Sylfirm X combination labelled for inflamed skin for that stage.

02Alternatives

Other scar tools are considered alongside the vial

The clinic's records list acne scars and pores among the aims of its Pico K laser, and pores and acne scarring among the concerns for Juvelook. If either suits your scars better than a vial over the pass, the plan says so.

03Surgeon

A board-certified plastic surgeon reads the scars

Dr. Jonghwan Choi decides which zones are treated, at what depth and with what, and reviews the same skin afterwards. Scar depth is judged by the person who sets the needle, so there is no hand-off between assessment and treatment.

Assessment for scarring takes place with the face clean and lit from the side, since shallow scars flatten under direct light. The doctor maps each zone, notes any active spots, and asks about past acne medication, recent peels and anything injected into the cheeks. If the plan includes a vial, its sealed packaging can be opened in front of you before the pass. The pass is carried out zone by zone at the depths agreed, and the preparation is applied over the treated area afterwards.

Exosome and acne scars: what patients ask

Does exosome therapy help acne scars?
Not as a treatment by itself. Acne scars are structural, so the reshaping is done by a device such as Sylfirm X working at depth. The exosome vial can be added over the pass, but published work on the preparation is review-level and the reviews cited here are not scar trials.
Is exosome combined with a device for scar treatment?
Yes, always at this clinic. Rewind's price sheet lists the exosome vial only together with Sylfirm X radiofrequency microneedling. The pass is set to the depth of the scars in each zone, and the preparation is applied over the treated skin afterwards.
Can exosome treat active acne?

Published work on acne scars, microneedle RF and exosomes

The first three papers study fractional microneedle radiofrequency for acne and atrophic acne scarring, which is the device half of this plan. The last two are reviews of exosome products: one covering current applications and clinical evidence, one covering complications. None was carried out at Rewind Plastic Surgery or funded by it, and none can tell you how your own scars will respond.
