Exosome with RF microneedling: why the vial follows the Sylfirm X pass

The short answer

Exosomes are applied after RF microneedling because the needling pass is what gives them a way into the skin. Sylfirm X sends fine needles through the surface and releases radiofrequency heat at their tips, leaving a field of open channels behind. The extracellular vesicle preparation is spread over that field while the channels are still open. Applied to unbroken skin, the same vial would mostly stay on the surface.

The two halves of this treatment have very different evidence behind them, and that difference should shape how you think about the pairing. Fractional microneedle radiofrequency has a clinical literature of its own, including a real-world series of 75 patients treated for enlarged pores and a 2022 meta-analysis on acne and acne scarring. The vial is supported by review-level work, and the reviews cited on this page were all published between 2024 and 2026.

So the useful comparison on this page is the Sylfirm X pass on its own against the same pass with a vial added. The device is the constant. What changes is one step at the end of the session, one line on the bill and one more question to ask at consultation.

What exosome adds to an RF microneedling session, and what it leaves unchanged

Most of what happens in this combination is the device. Separating the device's contribution from the vial's is the clearest way to judge whether the addition is worth it for your skin.

01Needle

The needle's work is identical with or without the vial

Sylfirm X carries microneedles adjustable from 0.3 mm to 4.0 mm, according to Rewind's equipment record. The depth is chosen for the condition being treated, and adding a vial afterwards does not alter that choice or the number of passes.

02Heat

The heat is delivered at the tip, below the surface

Radiofrequency energy is released where the needle stops, so the layer chosen is the layer heated. The clinic records melasma, redness, pore appearance and firmness as the aims for this platform, and all four are device aims before any preparation is involved.

03Vial

The exosome step is the only thing that differs

Once the pass is finished, one vial of extracellular vesicle preparation is spread across the treated area. The idea behind the pairing is access: the channels are open for a short time, and the preparation is applied in that window. A 2026 scoping review of topical products is the place to read how the claims made for this step compare with the clinical data.

04Same

Recovery rules do not change

Rewind publishes one aftercare sheet for Sylfirm X and it applies whether a vial was used or not. No shorter recovery is promised for the combined session, and a published review of complications after exosome treatment is a reminder that an extra product is also an extra variable.

The session runs in a fixed order. The skin is examined and cleansed, the doctor confirms the depth and energy for each zone, and the Sylfirm X pass is carried out. If a vial is part of your plan, you can ask for its packaging to be opened in front of you, and it is then applied across the treated area straight after the pass. What the doctor needs from you is a clear account of anything used on the same skin recently: retinoids, peels, other devices or injected products. A needling pass over skin that is already irritated behaves differently, and that is true whatever is applied afterwards.

Expected effects of the pass include redness, swelling, pinpoint bleeding, bruising and itching. Infection is uncommon. Adverse reactions after exosome products, some of them inflammatory, are described in the complications review listed at the end of this page. Worsening pain after the first day, spreading redness, pustules or a fever mean you should contact the clinic without waiting for your review.

Sylfirm X and exosome: questions about the combination

Why is the exosome applied after the Sylfirm X pass?
The pass creates open channels in the skin, and the preparation is spread over them while they are still open. Intact skin is an effective barrier, so the same vial on untreated skin would largely remain on the surface. The order is the whole point of pairing the two.
How many Sylfirm X plus exosome sessions are recommended?
Rewind publishes no fixed number or interval for this combination. After the first session the doctor reviews how the skin responded and decides whether a second one is useful, and whether it needs the vial or only the device. Any estimate you hear before that review is a guess.
Does adding exosome shorten the recovery after RF microneedling?
The clinic makes no such claim. The same aftercare sheet applies with or without the vial: no washing for seven to eight hours, a week without sauna, swimming or exfoliation, and daily sunscreen. Plan your schedule around the pass itself.
Is RF microneedling worth doing without the exosome step?
Often, yes. The device carries the better-documented part of the treatment, and Sylfirm X on its own is a listed plan at Rewind with a standalone rate of its own. The vial is added only when the doctor can give a reason for it in your case.
Does the needle depth change when exosome is added?
No. Depth is set between 0.3 mm and 4.0 mm according to the condition being treated, and that decision is made before the vial is considered. Adding a preparation afterwards does not alter the pass itself.
How much more does the exosome step add to a Sylfirm X session?
The device drops from its standalone rate of ₩350,000 to its combined rate of ₩250,000, and the vial component is added on top. The exact vial figure is written on your quote, and VAT applies to the total.
Can a different preparation be used after the pass?
Yes. Rewind's price sheet lists more than one vial paired with Sylfirm X, including one labelled for inflamed, acne-prone skin. Which preparation, if any, is used depends on what the examination shows, and the product is named before the session.
What are the risks of combining the two?
The pass brings redness, swelling, pinpoint bleeding and occasional bruising, and infection is uncommon but possible. The vial adds its own variable; published reports describe reactions after exosome use, including inflammatory ones. Tell the doctor about any past reaction to products applied after needling.

Research behind the pass and the vial

The first two papers concern fractional microneedle radiofrequency, the device half of this combination. The other three concern exosome products: a scoping review of commercial topical preparations against their clinical evidence, a systematic review of vesicle-based facial rejuvenation, and a review of complications. None of them studied patients at Rewind Plastic Surgery or was funded by the clinic, and none describes what your own skin will do.
