What is the difference between a neck lift and a deep neck lift?

The short answer

At Rewind the two names describe different layers. A neck lift treats loose skin and a slackened platysma, the muscle sheet at the front of the neck, by redraping and tightening them. A deep neck lift at Rewind is the clinic's grape-cluster fat removal: fat that clings in granules beneath the platysma is taken out by hand through a line of about 2 cm under the chin, and the muscle is then tied back. No skin is excised. Only the deep operation reaches the fat beneath the muscle; the digastric muscle is named in Rewind's published description of neither operation.

The deep neck lift description comes from Rewind's Korean surgical page for grape-cluster fat removal. Rewind publishes no technique page for its neck lift, so its incisions, anaesthetic and theatre time are confirmed by the surgeon at the examination.

The confusion is understandable, because in English the phrase deep neck lift is used for more than one thing. Abroad it often describes a large operation that opens beside the ears, redrapes skin and works on the structures under the muscle all at once. Rewind uses the name for a narrower operation through a short line under the chin, centred on the fat beneath the platysma and on the muscle itself.

So the useful comparison is by layer. Loose skin belongs to the neck lift. Fat under the muscle belongs to the deep operation. A slack platysma can be tightened in either. Some necks need both, and Rewind lists the deep fat removal as an addition so that the two can sit in one plan.

01Skin

Loose skin: the neck lift's territory

Only the neck lift redrapes skin and removes the surplus. The deep operation leaves skin in place and relies on it to settle over the slimmer outline, so good skin quality matters for it.

02Fat above

Fat above the muscle: part of either plan

Rewind's deep operation begins by softening and suctioning excess fat above the muscle. Where a neck lift is planned, the fat above the muscle is assessed at the examination and treated if needed.

03Platysma

The platysma: tightened in either operation

Both operations can tie back a slack muscle. In the deep operation it is closed after the fat beneath it has been removed; for the neck lift, the surgeon confirms the method.

Granules of fat stuck to the underside of the platysma cannot be drawn through a cannula and are left untouched by redraping skin, so they are taken out by hand. Published work has used ultrasound before surgery to measure this layer and predict how much will be removed.

The digastric muscle and the gland

Below the fat lie the paired digastric muscles and the submandibular glands. Rewind's published description of its deep operation names the fat and the platysma but does not name the digastric, and gland reduction is a separate listing. Whether any deeper structure matters in your neck is judged at the examination.

The deep operation uses a line of about 2 cm under the chin and a week in compression. For the neck lift listing, the incision plan and the dressing schedule are set by the surgeon.

Two operations with similar names invite the assumption that one is simply a stronger version of the other. They treat different layers, and keeping them apart keeps each decision clear.

Skin that lingers when lifted points to a neck lift. Fullness that stays after the fat above the muscle has been pinched and accounted for points to the deep compartment. The examination looks for both.

Deep fat removal is listed as an addition

Rewind prices the removal of fat beneath the muscle as its own line, so it can be added to a plan and seen on the quote, or left out when the compartment is not loaded.

Which neck suits a neck lift, which suits a deep neck lift, and which needs both

A neck lift is the likelier answer when

  • Skin hangs or folds below the jaw and is slow to return when lifted
  • The neck is slim and the problem is mostly laxity
  • Bands and a soft jaw border come with loose skin
  • Earlier fat removal left the covering slack

The deep operation, both, or neither when

  • Fullness stays under the chin once the pinchable fat is accounted for, and the skin still holds: the deep operation
  • A structurally short chin pushes soft tissue forward, which Rewind names among the reasons for the deep operation
  • Deep fullness and loose skin appear together: a plan may use both
